Meredith, a 64-year-old woman with relapsing MS has been treated with DMT for more than 20 years. She has a history of well-controlled hypertension and osteoarthritis. During her last visit to the clinic, Meredith requested a discussion about discontinuing her DMT. She admitted to the nurse that ever since her recent experience with community-acquired pneumonia, she has become increasingly concerned about the risks of continuing DMT. Before discussing the topic further with Meredith and making a recommendation, the health care team reviewed her clinical history and her most recent MRI to evaluate the potential risks of disease reactivation versus the benefits of discontinuation. The last MRI was stable with no new or enlarging T2 lesions and no contrast-enhancing lesions.
